welcome my name is Cindy Kozlowski and I am here to guide you through the transition process from preschool special education to transitional kindergarten t k or kindergarten it is important for you to know there are age-related guidelines which will help determine a child's placement for the following school year whether it be PK or kindergarten you may have gone through a previous transition process from early start to preschool special education name some services or this may be the first time you are going through a transition process for your child the steps in the preschool special education to t k or kindergarten trans send process include planning for transition transition assessment parent observation of programs and Trey session individualized education plan or i e p meetings along with reviewing these steps I will explain your important role as the parent or caregiver in this transition process and as an important member of your child's IEP team I went through this transition process with my own child and I know it may seem overwhelming be assured you have a team of about to support you through this process along with other special education and Community Resources I will talk about planning for transition any child who hasn't BP and is receiving special education services will have a transition meeting before they end preschool Services you may not be aware but your child IEP team starts in formal planning for the transition to t k or kindergarten at the beginning of the school year the transition process your parent questions concerns input and general transition is often discussed at parent-teacher conferences during the school year as well as I EP plan review meetings that occur at the start of the school year next comes the transition assessment after winter break your child Special Education team begins to discuss your child need to make recommendations 40K or kindergarten placement and special education services placement recommendations are based on your child's progress on their IEP goals observations of your child desired results developmental profile drdp and input from both you the parent and the special education service providers working directly with your child in some cases it may be appropriate to receive input from a child's general education teacher some children will have re-evaluations in all or some Developmental and pre academic areas depending on their progress toward goals as well as teacher and or specialist concerns some examples of the specialist who may be on your child's team are speech therapist occupational therapist adaptive call education teachers an establishment may be conducted if your child is due for a special education eligibility evaluation or if their team has questions and needs more data to make placement decision for the following school year if a formal assessment will be completed you will receive an assessment plan to provide your consent for the assessment as a reminder the formal assessment will be completed without a parent's consent if the team is an agreement that they have the necessary data to make recommendations 40K or kindergarten placement and special education service has been the team may decide to collect informal assessment data observations and work samples are examples this would be instead of conducting standardized testing to make their record station 40 K or kindergarten placement and special education services for the following school year in the spring parents may go for observations of specific recommended programs this must be requested by youth the parent observations of any recommended programs typically last approximately 20 minutes and will be coordinated by one of your current School District team members transition i e p meetings are typically held after winter break do most take place between February and May the transition meeting May her as part of your child's IEP review or separately as an amendment to your child's IEP Apple transition meeting your child's current case manager which might be their special education teacher or primary service provider they will be in attendance as well as any other special education and general education providers to make recommendations there may also be what three senators from the school your child will attend 40K or kindergarten in attendance during the transition meeting any updated assessment will be discussed current goals will be reviewed special education services 40K or kindergarten will be discussed if necessary sometimes if the IEP team is an agreement and the child is staying in the same program or transitioning to general education with a service they are already receiving such a speech therapy and amendment can be set home for your signature parents will be consulted and must be an agreement weapon on meeting Amendment the need for transportation will also be discussed as part of the transition i e p meeting typically if your child is eligible for transfer case door-to-door transportation is offered to students with moderate-to-severe needs students in mild to moderate special day class programs usually transition from door-to-door transportation in priests to a central pickup 40K or kindergarten Central pickup means that Transportation will be provided to and from one central location such as from your neighborhood Ella Mae preschool to and from the school where your child will be attending the special day class program you the parent as an important member of your child's team also has responsibilities one is to provide input your child's team during the transition process and to attend that transition IEP meeting or review the non meeting Amendment you will also authorized and approved that I EP or an amendment by signing and returning it to your child case manager please make every effort to attend the transition BP meeting as scheduled it is important to communicate any schedule conflicts or confirm meeting with your child case manager you must also register your child 40K Ork in instructions for registering your child will be provided by your school district at the transition i e p meeting or by your child's current case manager school registration who is typically completed online once register you might be contacted by school office staff to provide additional documentation such as updated Indiana station prior to starting school if you have any questions or concerns about the transition process please reach out to your child current case manager they can help to guide you through the process as well as address specific needs or questions that you may have the Santa Clarita Valley special education local plan area or South Bay website as indicated on your screen under the resources tab is another wonderful resource to help you through the transition process if your child is a North Los Angeles County Regional Center consumer you may also reach out to a child's consumer service coordinator to determine how the transition process into t k or kindergarten with the school district may impact Regional Center services last the Family Focus resource and Empowerment Center as indicated on your screen can provide parent to parent support resources and information on the special education 2tk for kindergarten process in closing we want you to know
