welcome my name is Cindy Kozlowski with the Santa Clarita Valley special education local plan area or scalp and I am here to guide you through the special education transition process from Junior High to high school the information I am presenting was put together by the special education at the William asked Hart Union High School District and I am grateful for their contributions I went through this transition process with my own child and I know it may seem overwhelming even if you have previously gone through a special education transition with your child and the school district for example from elementary to Junior High School be assured you have a team of able to support you through this process along with other special education and Community Resources you will hear the word transition and you might wonder transition it means moving from one state to another it is a process that happens over time not just occurring on one specific date you will also hear that term matriculate matriculate means advancing in school students transition or matriculate from preschool to kindergarten Elementary to Junior High School Junior High School to high school this can be an exciting time as your tile transitions from Junior High to high school and it can also be a concerning time for students and Families your child may look forward to increased Independence making new friends and participating in new activities some concerns may include unfamiliar people new expectations new role and a more social atmosphere in high school are you watch your child continue to grow socially and likely you will also see them experience new changes and Milestones during this stage of Life personal changes are taking place physically emotionally and socially these changes might magnify other challenges your child experiences during this time this video will help you learn what to expect as your child transition from Junior High to it will also help you gain an understanding of the transition matriculation process partnership and best supports and resources available to students and their families during the transition from Junior High to high school just as all of the students in your child's Elementary School moved into a specific Junior High School most of the students in your child's Junior High School will move into a specific high school campus this is usually the school closest to your home sometimes called your precedence or your homeschool however some students may have more specialized needs some examples of this would be academic needs environment access to classroom or School environment unique medical needs for healthcare status they may transition to a high school that provides those Services which are not available after School Apple residence parents or guardians of children and the Heart District may also apply for an intradistrict transfer to request attendance at a school other than their school of it's within the heart district there are specific guidelines for intra-district transfer has to be granted if you would like a digital information about intra-district transfers it is a bubble under Hart District website or you can call the student services department with hartdistrict we will provide you with the website and phone number at the end of this video now let's review the transition matriculation process which begins when your child is in their 8th grade year during this time your child teachers at the gym your high school share information about your child individualized education program or IEP with the high school this allows the high school to anticipate the number of students they can expect in the upcoming year it also helps staff to plan for matriculation i e p meetings to be held in the spring of your child 8th grade school year if your child attends Cascade Middle School before sharing your child's information with the Hart district has stay at Union School District will ask you to sign a document called a mutual exchange of information an authorization this allows cat day at Union School District to share information about your child IEP with the Hart District if your child is attending Junior High the Hart District they will remain with the same school district for high school so no Mutual exchange of information authorization needs to be signed matriculation AP meetings provide an opportunity for parents or Guardians to meet some of the staff members from the high school who will be part of their child's IEP team the meetings are typically scheduled in the bring up your child 8th grade school year this is prior to your child transition from Junior High School to high school you should plan for the matriculation i e p meeting last about 30 minutes unless otherwise stated on your IEP meeting appointment notice or by your child's case manager special education teachers and service providers from both the junior high and high school will be present at the matriculation i e p meeting your child's current Special Education team from Senior High School will share information about your child and their IP with the high school staff present at the meeting the high school teachers and service providers will also be there to answer questions as to how special education services will be provided at the new school for example if your child currently receives what are called related services such as speech and language therapy or occupational therapy a representative from that specialty at the high school will be present at the matriculation i e p meeting parents and Guardians also have responsibilities during this process you must attend the matriculation IEP meeting you will be notified by your child case manager which might be their special education teacher or primary service provider of the proposed matriculation IEP meeting date and time with the Hart District it is important to keep the schedule time if you need to change the meeting for any reason please let your child's case manager know right away temperature patient i e p meetings may be held in person at the high school or by teleconference such a zoom or Google meet it is encouraged for your child to attend the matriculate i e p meeting with you just be sure to let their case manager know in advance if your child is attending the meeting think about questions you may have regarding your child transitioning High School you are encouraged to write down your question so you won't forget them during the meeting a sample agenda for the matriculation IEP meeting and call the purpose of the meeting introductions of attendees review of your child's eligibility for special education and current Junior High School related supports and services some examples include resource specialist program assistive technology speech and language therapy and station description of the high school special education programs as well as related supports and services input and discussion within your child and as a reminder you the parent or Guardian are a very important part of that Team all of whom will be participating in the matriculation i e p meeting this includes a discussion at the best placement options based on your child's knees at that time if requested visitation and or observations of programs will be arranged within the team offer of free and appropriate public education or vape and related supports and services percentage of time your child will be in the general education setting versus receiving special education support or services in a setting outside of the general education classroom we understand that this time of transition for your child can be both exciting and concerning at the same time it is important to have open communication with your child and with their IEP team if you have questions prior to the matriculation IEP meeting you can contact your child's current case manager or the Hart District metric patient coordinator as a reminder if you are in Castaic Union School District along with your child current case manager it would be the casting middle school but trick patient coordinator or the heart District matriculation coordinator you will find that your child new school may have some additional support such as full Round-Up sore hip hop events these events allow your child to become familiar with their new school and meet new friends so be sure your child attend some program this may offer meet-and-greet event to allow your child time to meet some of the school staff they'll see on campus your child will receive their class schedule close to the first day of school for the School District student portal as mentioned previously if you have any questions or concerns about this transition process please read talk to your child case manager School administrators and School District program specialist can also assist with answering questions additional reach out to your child consumer service coordinator to determine how the transition process into high school with the school district may impact Regional Center services lastly the Family Focus resource and Empowerment Center as indicated on your can provide parent to parent support resources and information on the special education Junior High to high school process remember collaboration communication and building relationships contributes to successful transitions for students including your child as the parent or Guardian are a very important part of your tiles team and in the words
